With increasing age, the number of medications rises.
Many people take five, seven or more medications daily, often simultaneously.
The more active substances overlap, the higher the risk of drug interactions.
Approximately 4–7% of all hospital admissions in Switzerland are associated with medication-related problems.
This is not an exception.
This is a structural problem.
Information exists – but it is distributed across different lists, systems and stakeholders.
